The severity of pain varies with each man but it has nothing to do with being erect and everything to do with your body getting ready to ejaculate. The most interesting part of this thread was the vehemence with which the women were arguing over how the penis behaves, while men conteded it was not so. Anyone who believes they have found documentation in a medical text claiming that urination while erect is either impossible or normally painful has misunderstood what they read. I would bet that there is not a man alive over the age of 13 who has not woken up at least once with an erection and a full bladder. As others pointed out, the difficulty is in the aim, not the physical act. If this was some amazingly unusual phenomenon it would not have featured as an entire scene in "40 Year-Old Virgin", and the men in the audience would not have been laughing so hard :)
